Getting Started
Patient registration is the first point of contact between all patients and Charles Moser, PhD, MD. It involves the completion of various forms and documents, including patient intake forms, consent forms and financial responsibility forms. Patients must send an email to Charles Moser, PhD, MD to request the patient registration forms and receive directions for how to complete the forms and submit the forms back to Charles Moser, PhD, MD. All communication regarding patient registration must be done via Charles Moser, PhD, MD’s HIPAA compliant email.
Request an Appointment
Patients who have completed the patient registration process (see Getting Started above) can request an appointment by sending an email to Charles Moser, PhD, MD. All communication regarding appointments must be done via Charles Moser, PhD, MD’s HIPAA compliant email.
Make a Payment
Charles Moser, PhD, MD accepts credit cards through Square. Please note that if the Square checkout screen attempts to add a “tip”, then select “Other” and change the tip to $0.00 as Charles Moser, PhD, MD does not accept nor collect tips.
